Procedure
Mail or In-Person Requests: Mail or present in person the original notarized document, along with the fee and a completed “Request for Authentication” form.
Request for Authentication form: A “Request for Authentication” form is available for download on the Secretary of State’s website. The form includes all the information needed to process the request.
Fees: $10 for a certificate authenticating the Notary’s status, including an Apostille, payable to “State of Oregon” or “Corporation Division” (OAR 160-100-0040[1] and [2] and website, “How to Get an Authentication (or Apostille)”).
Delivery of Documents: If return by mail is desired, include “a self-addressed stamped envelope or pre-paid, pre-printed label from FedEx or UPS” (website, “How to Get an Authentication (or Apostille).”
Unauthorized Authentications: “The Secretary of State may not certify a signature of a notary public on a document:
“(a) Regarding allegiance to a government or jurisdiction;
“(b) Relating to the relinquishment or renunciation of citizenship, sovereignty, in itinere status or world service authority; or
“(c) Setting forth or implying a claim of immunity for the bearer from the laws of this state or nation” (ORS 177.065).
